In fact, thyroid hormone levels in the weeks before conception may be among the most consequential of the entire pregnancy. The developing embryo relies entirely on maternal thyroid hormone during the first trimester — the fetal thyroid does not begin functioning independently until approximately weeks 10 to 12 of gestation.

For this reason, most endocrinologists and maternal-fetal medicine specialists recommend that women on Synthroid achieve a TSH level below 2.5 mIU/L before attempting to conceive. Some guidelines suggest aiming for a TSH closer to 1.0 to 1.5 mIU/L, particularly for women with a history of miscarriage or impaired fertility.
